Job summary An opportunity for a 6 month secondment post has arisen for a motivated, dynamic person to join our senior team in our Older Persons Rapid Access Clinic and Frailty Unit at the Ealing Hospital site. You will need to be able to support the junior team as well as the senior team and take charge of the unit in the absence of the ward manager. OPRAC /Frailty is part of the Integrated Medicine division. We care for patients from GP and Hospital referrals. We manage patients with Parkinsons disease, Heart Failure, Anaemia, Osteoporosis and various other medical symptoms. The team work collaboratively with Consultants, Medical teams, Specialist Nurses and Pharmacists to deliver individualized patient focused care. Please note- the working pattern for this role is Monday to Friday between 8.00 am to 6 pm. OPRAC /Frailty is based within the frailty unit and the successful person will be expected to work with the multidisciplinary team to provide early and pro-active Comprehensive Geriatric Assessment (CGA) to patients who meet the referral criteria when the service requires. The holistic assessment will include clinical, social, functional, emotion and cognitive elements as documented within the CGA..
