Ilkeston Community Nursing Team
Ilkeston community nurses provide core nursing care across the north of Erewash locality while working closely with the Rapid nursing team and wider PCN.
This is an ideal opportunity for a qualified specialist practitioner (District Nurse) with varied community experience who wants to enhance their leadership and caseload management skills supported by other District Nurses and a dedicated Clinical Lead District Nurse.
Main duties of the job
Experience in caseload management with a high standard of community nursing knowledge and clinical skills will be required.
You will be provided with training and support tailored to your needs to drive forward high-quality care and teamwork including a thorough induction programme, regular one-to-ones, peer supervision, working within an MDT, and clinical training.
Provide effective evidenced-based care to a defined population/Primary Care Network. This is achieved through effective delegation, caseload management, monitoring of care standards, leadership, and the performance management of team members.
To assess, plan, implement and evaluate individualised patient care, utilising your professional judgement at all times.
Provide clinical leadership and line management to Registered Nurses, Nursing Associates, and other members of the multidisciplinary team in the delivery of high-quality care.
To contribute to the continuous improvement of community nursing in line with the NHS long-term plan.
