As a Registered Nurse on our Rapid Response Team, you will work under the leadership of a Senior Registered Nurse and collaborate with a multidisciplinary team to provide high-quality, evidence-based specialist palliative care and clinical advice. You will be registered with the NMC, with experience in specialist palliative care preferred. In this role, you will also liaise with the Clinical Nurse Specialist and wider community teams to ensure patients are appropriately prioritised.

We’ve expanded our area over that time and now cover over 1,000 square miles of our county including Exeter and Central and East Devon. If you’ve been diagnosed with any type of terminal illness, our expert doctors and nurses are here to help. Our services include support at home, a specialist palliative care ward, outpatients clinics, and supportive therapies.
We support over 2,000 people each year, in some of the hardest times possible. With just 18% of our funding coming from the NHS, we need to raise around £10 million a year to continue to provide our care, relying on the generous support of local people and communities.

Develop knowledge and skills to be able tocontribute in the delivery of education and learning for all those involved inpalliative/end of life care, in order to act as a resource to widerprofessional and care teams both internally and externally.
Contribute to Hospiscares clinical governance andaudit process in order to support continuing quality improvement and learning.
Develop advanced skills in identifying andassessing complex bereavement and communicate effectively with the Supportive Careteam in order to enable a seamless handover into Hospiscares Bereavement CareServices.
Co-ordinate junior colleagues and takeresponsibility for the team in the absence of the CCTL or senior practitioner toensure continuity of service and welfare of patients.
Facilitate the planning and implementation of thesafe transfer of care to other health and social care providers

Experience
* Demonstrable experience of assessing, planning and evaluating treatment and nursing care plans
* Demonstrable experience of caring for patients with life-limiting conditions
