Hillsborough Network are looking to recruit a Care Co-ordinator to work with Tramways & Middlewood Medical Centres in the Hillsborough area of Sheffield. This will ideally be a full time position or between 30 - hours per week. There maybe some weekend working.
You will become a member of a very friendly, experienced and supportive team of clinical and non clinical staff in modern sites serving over 10,900 patients.
We are looking for a highly motivated individual who can integrate with multi-disciplinary teams within the network to deliver effective, co-ordinated care for our patients. You will need to be able to work with and understand the roles of a variety of different people working in the practice and across other health care settings.
You will be contributing to tackling health inequalities in health and social care particularly regarding individuals with long term conditions including learning disability and dementia as well as cancer patients.
Main duties of the job
1. working with the Network Team to ensure that the requirements of the Network DES Service Specifications, Investment and Impact fund and Quality Outcome Framework and quality indicators are met.
2. the applicant will also be supporting the network with the co-ordination and delivery of the Covid Vaccine, RSV and annual Flu vaccination programmes.
3. provide coordination and navigation for people and their carers across health and care services, developing excellent working relationships with all partners, wider services within the network.
4. using excellent communication and organisational skills to bring together all of a persons identified care and support needs, and explore their options to meet these into a single personalised care and support plan.
5. Ensure that people have good quality information to help them make choices about their care, health and wellbeing.
6. Manage and prioritise workload on a daily basis and deal with the competing demands of the MDT

By using funding from Primary Care NetworkDirect Enhanced Service have also been able to offer our patients additionalservices such social prescribing, first contact physiotherapy and access to ourpharmacy team.
This position will be based within Tramways & Middlewood Medical Centres where we have plenty of free parking, access to NHS pension scheme and free refreshments.

Job responsibilities
The Care Co-ordinator will:
11. utilise population health intelligence to proactively work with the MDT to ensure they are aware and notified of patients that would benefit from access to personalised care.
12. work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN.
13. raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations.
14. bring together all of a persons identified care and support needs from digital information, and explore options to meet these into a single personalised care and support plan, using excellent communication and organisational skills to liaise with other stakeholders as needed for the collective benefit of the patients.
15. help people to manage their needs through answering queries, signposting to relevant services and ensuring that people have good quality written or verbal information to help them make choices about their care.
16. Identify patient need and assist to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.
17. signpost people to gain access for personal health budgets where appropriate.
18. provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care professionals.
19. Ensure awareness of and basic safeguarding processes are in place for vulnerable individuals.

Care Co-ordinator Responsibilities:
26. Ensure all mandatory training is completed on an annual basis including; safeguarding, confidentiality, equality and diversity, Cardio-pulmonary resuscitation etc.
27. Ensure infection control guidelines are maintained.
28. Work with a supervisor to take responsibility for developing own practical and theoretical competence, developing own reflective practice skills.
29. Contribute towards developing a culture of continued learning and innovation, supporting continuous improvements in care delivery.
30. Adhere to legislation, policies, procedures and guidelines both local and national, regularly attending workplace and staff engagement meetings.
31. Maintain accurate and contemporaneous patient health records
32. Work in an effective and organised manner demonstrating excellent time management and organisational skills to effectively deliver person-centred care for an allocated group of individuals

Person Specification
Qualifications
Essential
33. GCSE Grade A-C in Maths and English or skills level 2 Maths and English or equivalent
34. ECDL or equivalent
35. Diploma /HNC level education or relevant experience.
36. NVQ level 3 Business administration or relevant experience
Desirable
37. Commitment to Continued Personal Development through further study.
38. Long term conditions training/awareness
39. Welfare Rights basic training
40. Qualified and proven comprehensive knowledge of Micosoft packages including spreadsheets.
