Job overview
An interesting opportunity has arisen for a year-long secondment to our established team as a Band 6 Mental Health Practitioner. The post provides a good work-life balance, working 9-5, 4 days a week (30 hours total). We care for Older People based in the coastal communities of Suffolk.
Our friendly and supportive team provides a community-based service to people of any age with a dementia diagnosis, and to Older People with a functional diagnosis and complex needs.
We are proud to be accredited with the Royal College of Psychiatrists as providing high-quality care and good practice. This is important to us, as is our staff wellbeing; we have a Wellbeing Champion in the team and make sure we meet up for Wellbeing Lunches, have away days as well as occasional social nights out.
We are looking for someone who is interested in working in Older People’s services, has an interest in physical as well as mental health and who has an understanding of assessment, the referral process, case handling and working collaboratively with the wider MDT.
** This is a temporary position to provide cover for a year, while one of our established team members is on secondment with another team. **
Main duties of the job
As a Community Mental Health Practitioner, you will be responsible for:
* Holding a caseload of patients with organic and functional presentations under CPA and NCPA.
* Assessing and managing risk.
* Using Dialog+ where possible and Recovery Care Planning with service users and carers.
* Managing your own diary and planning your day.
* Attending MDT meetings and ward review meetings.
* Duty working 2 half days per week.
* Communicating effectively through notes on Lorenzo and letters to GPs and other involved parties.
* There is an opportunity to learn to take ECG readings and to learn phlebotomy.
* Driving is essential for this role.
* Full induction will be given.

Detailed job description and main responsibilities
The team operates from Monday to Friday from 9am to 5pm. We support good work/life balance and promote hybrid working. Professional meetings, telephone contacts and administration work can be completed remotely but we require office attendance to cover duty work and face-to-face appointments for the service users.
You will be part of a highly performing multidisciplinary team of clinicians with strongly embedded values of working together. Our team consists of psychiatrists, a psychologist, a CAT therapist, a Clinical Assistant Psychologist, an OT, mental health nurses and two assistant practitioners. We are diverse in background, profession, and experience. In March 2023 the team achieved national accreditation with the Royal College of Psychiatrists in recognition of the high quality of care it provides for service users and their carers. You will be responsible for managing a caseload, assessing and planning treatment for your patients. You will work with the patient and any carers to plan care and optimise treatment plans. You will feed back to the MDT and work within the team for the best outcome for the patient. We are a kind and supportive team and will fully support you in this busy and demanding role.

Qualifications
* Nursing degree or equivalent, Dip OT, Clinical Psychology or equivalent.
* Ongoing professional development working towards degree level.
Experience
* Experience in the delivery of clinical interventions.
* Experience in working in a team delivering person-centred treatment packages.
Skills
* Effective analytical and decision-making skills.
