BLMK ICB is looking for a senior Nurse who is experienced in assessing a variety of children's health needs, demonstrates leadership qualities, and has a proven track record managing highly complex cases. You will be working as part of a small, friendly, dedicated team who oversee the processes required for BLMK ICB to make recommendations relating to funding packages of care proportionate to need.
The post holder will also work with the team to ensure that BLMK ICB meets national guidance and requirements relating to its commissioning responsibilities. They will work closely with the lead provider to ensure that commissioned packages of care align with evidence-based practice and the National Children's Continuing Care Framework, SEND, and Transforming Care.
Main duties of the job
This role is responsible for developing, managing, and implementing the processes required for BLMK ICB to make recommendations relating to all funding requests for children and young people (CYP). This currently includes CCC - Physical, Mental health, Learning Disability, and Autism as well as Personal Health Budgets.
The post holder will ensure that BLMK ICB meets national guidance and requirements relating to BLMK ICB's commissioning responsibilities, working closely with the lead provider to ensure that commissioned packages of care are in line with evidence-based practice and the National Children's Continuing Care Framework, SEND, and Transforming Care.
As a member of the Continuing Care team, the role will include working across our four Local Authority areas as well as potential visits to clients that live outside of the region.
The CCC team operates a hybrid working model that supports flexibility in its approach to sustaining a workforce to meet the service needs, including home and office working as agreed with the wider CYP Integrated and Personalisation management team.

Job responsibilities
* Be responsible for the Children's Continuing Care assessment of children and young people with Mental Health/LD needs and Physical Health needs.
* Triage and prioritise referrals and communicate the outcome of the referral.
* Be involved in the planning, development, and review of care.
* Be involved in Care, Education, and Treatment Reviews (CETR) and Local area emergency protocols (LAEPS).
* Act as a resource for families and other stakeholders regarding the eligibility and process of application for funding for children, including Children's Continuing Care, Personal Health Budgets, and other joint packages of care.
* Build local and regional networks to ensure expert health advice informs ICB/CCC processes.
* Review and assess children and young people as per the Children's Continuing Care Guidance and report any changes required within the care package to the multi-agency eligibility and provision panels.
* Utilise expert skills in assessing a child's condition, including caring for children with complex disabilities, behavioral management, and the equipment needed for the child.
* Have strategic oversight of complex discharges from tertiary and acute centres.
* In conjunction with the Case Manager, build strong links with the lead provider and other agencies, ensuring that families receive the most appropriate and equitable service.
* Initiate co-production to ensure children/families are involved in the planning and developing of their own care.
* Ensure robust processes for triaging of referrals, DST assessments, and reviews as well as escalations for CCC framework implementation.
* To maintain accurate and up-to-date records for auditing purposes.
* Actively involved in auditing of nurse assessors' record keeping.
* To act as a link person to coordinate services for the individual while a package is being developed and deployed or during a change to the package of care.
* To contribute to the implementation of safeguarding procedures.
* Work with local systems across all care settings to ensure that placements are both appropriate and fit for purpose.
* To provide assurance of quality and hold accountability of assessment during the verifying of a decision support tool assessment process.
* To provide assurance to the Eligibility and Provision Panel that the proposed package is collaborative and proportionate to need, equitable, promotes family resilience and independence.
* As part of local resolution, identify when evidence has not been used correctly or gaps in evidence.
* Line Management of Nurse Assessors.
* To provide induction to new team members supporting them through the induction pack and their probation competencies.

* NMC or equivalent Registration.
* Equivalent to NMC registration.
