Experienced Adult Social Worker

As an Adult Social Worker, you will be responsible for assessing the needs of clients, developing individualized care plans, and coordinating support services to ensure that our clients have access to the resources necessary for their well-being. This position requires a compassionate and proactive individual who can navigate complex social issues while working collaboratively with clients and their families. You will have the opportunity to engage with a diverse population, addressing challenges related to mental health, physical disabilities, and social isolation. Responsibilities
Conduct comprehensive assessments of clients' needs and situations
Develop and implement personalized care plans in collaboration with clients and other professionals
Provide direct support services, including counseling and crisis intervention
Facilitate access to community resources and services for clients
Monitor and evaluate clients' progress and adjust care plans as needed
Advocate for clients' rights and needs within various systems
Maintain thorough and accurate documentation of client interactions and services provided
Requirements
A Bachelor's or Master's degree in Social Work from an accredited institution
Current state licensure as a social worker
At least 3 years of experience in adult social work or a related field
Strong knowledge of social service systems and resources
Excellent communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Strong problem-solving skills and the ability to handle crisis situations effectively
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £35.00
Location: Liverpool, Merseyside
