Summary The RN Care Coordinator is responsible for day to day management of patient care from initial contact through recurring visits. Assures patient care services are timely and provided at the appropriate level of care. The primary goal of this role is to support and engage in activities that result in effective patient care and effective use of resources along the care delivery continuum. Minimum Education Graduate of an accredited school of nursing, BSN preferred. Minimum Work Experience 2 years related clinical RN experience per specialty area. Nursing charge or lead experience strongly preferred. Experience educating a diverse population on healthcare issues highly desirable. Required Licenses/Certifications Current Vermont RN license. BLS Certification through American Heart Association Valid Driver's License (if conducting home visits). Department specific Certifications may apply.
