As Deputy Team Leader, you will play a key role the effective management of the day to day running of a CQC registered care and support service within Mytholm Meadows Extra Care scheme (which Calderdale Council operates in partnership with Together Housing), with a specific focus on housing related support. Your role will also include assessing the needs of people we deliver care to, auditing, and maintaining positive relationships with colleagues, community professionals, the people we visit and their families. The scheme is designed to support older adults with care needs to live independently within a safe and nurturing environment.
In the role, among other duties, you will be:
• A key member of a multi-disciplinary, multi-agency team providing support to adults of all ages who live within the extra care setting
• Promoting and maintain individual’s independence, dignity and choice
• Undertaking personalised support to promote individual health and wellbeing within a community setting
• Ensuring the delivery of housing-related and wellbeing support to those living within the scheme
• Supporting tenants to settle into their new home
• Promoting the involvement of the wider community in the scheme
The ideal candidate will demonstrate a strong understanding of person-centred values and be able to put these into practice, have direct experience of providing care, communication and organisational skills, and have an NVQ level 3 in case (or similar).
Most importantly, you will have a positive approach - with a passion for supporting people’s independence, dignity, and well-being - that will enhance the experiences of staff and people living at Mytholm Meadows.
