Support the Dementia Outreach Service with the implementation of patient care plans, support and advice for care homes in the Broxtowe area. Monitor and record individuals' mental health status and report any significant changes. Assist individuals in developing and maintaining coping strategies to manage their symptoms. Seeing patients who are permanent residents in Care Homes within the Broxtowe area. Carrying out duty which involves making and receiving phone calls from care home staff offering support and guidance, reviewing their electronic records, and making entries. To contribute to assessment and interventions carried out by the dementia outreach. to engage with the Enhanced Health in Care Homes Service (EHCH) and with GPs in the local area. Part of a multi-disciplinary Outreach service which works with both Independent Sector and Statutory Sector providers of Residential Care to Older People with dementia. To increase the capacity, confidence, competence and willingness of those organisations to provide the right level of care to this vulnerable client group and by doing so to increase standards of care, improve quality of life, increase independence and choice. You will be supported through training and supervision. As this role is community based covering the Broxtowe area, a full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change.
