We are inviting applications for a highly motivated and dynamic Health Care Assistant to join North Tees & Hartlepool Community Nursing Service. This service will provide care to patients within a range of settings including patients' own homes and residential care homes.
The post holder will have a high level of knowledge in the field of community care and will have a specific interest in areas such as palliative care, long term conditions, catheterisation, and wound care. You will be required to work autonomously to provide a whole person approach to the care and management for patients within this setting.

Main duties of the job
The post involves liaising with a variety of health and social care providers and other agencies to ensure the provision of high quality care management to this group of patients and their carers.

Job responsibilities
We require a Health Care Assistant with NVQ level 3 or equivalent in health and social care who has excellent communication skills and is confident working across professional boundaries. You will need to be flexible, adaptable, and a good team player with the ability to work independently within a community setting. You will be prepared to work all shift patterns within the locality including early and late shifts, weekends, and bank holidays. There may also be the need to provide cover for the out of hours District Nursing team.
Person Specification
Qualification
* Good standard of literacy/numeracy.
* Able to use IT systems such as email and the internet to find information & the patient's electronic record.
Experience
* Demonstrate sound basic knowledge and understanding of a broad range of medical conditions, particularly those affecting older people.
* Evidence of ability to work with a multi-disciplinary team.
* Previous recent experience in a clinical setting.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
£24,071 to £25,674 a year pro rata, per annum.
