This is an exciting time to join our integrated team of Social Workers, District Nurses, Occupational Therapists, Physiotherapists, Community Mental Health Nurses, speech and language therapists, dieticians, experienced technicians and dementia support workers.
We are seeking to appoint an energetic, enthusiastic full-time, permanent community physiotherapist to this team. The team works with adults from the age of 18, into older age, affected by frailty and/or complex long-term health conditions and/or disabilities; often compounded by their mental health or emotional well-being needs.
The post holder will work closely with multi-professionals as well as GPs and Primary care staff to support individuals in achieving good quality emotional and physical well-being. They will be a key practitioner within the team, developing and providing effective, holistic and targeted support around the person.
Main duties of the job
* Establish rapport and gain consent from the person before applying professional skills.
* Communicate complex sensitive information in an understandable form to patients and carers.
* Share knowledge and skills with members of the multi-disciplinary team, peers, and students.
* Be clinically competent to provide advice and treatment to individuals with diverse healthcare needs.
* Undertake comprehensive physiotherapy assessments for a designated caseload.
* Be professionally and legally accountable for all aspects of your own work.
* Work with clients to identify physiotherapy goals as part of the overall care plan.
* Plan and implement individual and/or group interventions in collaboration with the client.
* Develop specialised programmes of care using specialised approaches and models of practice.
* Monitor and evaluate treatment to measure progress and ensure effectiveness of the intervention.

Qualifications
* Essential: Physiotherapy qualification and registered with HCPC.
Experience
* Essential: Experience of integrated working.
Skills
* Essential: Experience of assessments and managing complex conditions.
