Core Principles · Maintain current registration with the NMC register and uphold the NMC’s Code of Professional Conduct and their guidance at all times. · To continue professional development to maintain the highest level of competency required for your role through annual updates and any training as required. · Always adhere to practice and professional guidance, policies and protocols in relation to your role and the care of patients. · To always have patient care as the highest priority, through using your expertise and clinical skills to accurately assess patients’ individual needs and manage their treatment using practice protocols, PGDs and the appropriate guidelines. · Work with other healthcare professionals to monitor and manage patient conditions, care, monitoring and treatment. · Identify, manage, advise and support patients at risk of developing long term conditions, and deliver opportunistic health promotion. · Be responsible for the monitoring, implementation, and management of infection control for the surgery and health and safety practices within the nursing team alongside the Operations Manager. · Understand and apply policy that supports the identification of vulnerable children and adults, being aware of statutory procedure and local guidance. · Actively participate in QoF and Enhanced Services activities. · Actively participate in audit activity and policy and protocol development for the evaluation and development of surgery’s care and service to patients. · Produce accurate, contemporaneous, and complete records of patient consultations, consistent with the NMC and practice guidelines and protocols. · Support other members the team at all times both clinical and non-clinical, including mentoring and teaching where desired. · Take responsibility for their own work, as well as their learning and development at all times. · Ensure the maintenance of equipment and stock levels relating to patient care. · Comply with and follow the nurse revalidation programme. Job Responsibilities: · All immunisations and vaccinations, including childhood immunisations. · Child Health Surveillance. · Contraception and sexual health advice and management. · Travel Advice & Vaccines · Routine Injections · Hypertension and blood pressure monitoring. · Health checks · Cervical smear screening · Asthma and COPD monitoring and management. · Chaperone duties · Chronic Disease Management · Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals. · Understand the aims of management and the pharmacological treatments for chronic disease. · Initiate, perform and interpret diagnostic procedures as appropriate for chronic disease management. · Development of recall procedures, appropriate literature and health promotion, and audit. · Maximise QoF and Enhanced Services achievement and attend any training or updates necessary. Management and Administration · Provide and help to manage a caring, empathic and professional nursing and treatment service to the patients and practice team. · Liaise with other members of the surgery team and other agencies providing clinical care and support to patients within the local area.
