Community Alcohol Support Worker (CHAT)

Job overview
The Community Hospital Alcohol Team is a community-based team working closely with UHNM to provide alcohol support to the community of Stoke on Trent for patients that attend emergency portals with harmful use of alcohol. We reduce the risk associated with alcohol and promote change to improve the health and social welfare of patients. The community alcohol support worker (CHAT) will support the alcohol detox nurses within the team. This post is part time, 30 hours per week.
We offer a 7-day nurse-led service and our hours are 8am - 4pm providing increased support as required. We provide Psychosocial Interventions for up to 12 weeks from discharge at UHNM, focusing on harm reduction, relapse prevention, and seamless referrals to recovery services.
Main duties of the job
1. Support earlier discharge from hospital / admission avoidance for suitable patients with alcohol-related problems.
2. Assist Service Users who are utilizing multiple emergency services (e.g., ambulance, A&E, IOU, Harplands, crisis teams).
3. Provide increased support for service users with co-existing complex physical health needs (e.g., chronic liver disease, end of life) that require a coordinated case management approach.
4. Deliver increased support for people known to be difficult to engage in services who require an assertive management approach.
5. Assist Registered Professionals in assessments and care planning/implementation and evaluation of individualized care plans.

Detailed job description and main responsibilities
1. Communicate with service-users and carers, all members of the multi-disciplinary team, and staff in external agencies using a variety of methods including Information Technology.
2. Regularly report and effectively communicate service-users' health status and care aims in the Halo System.
3. Maintain professional boundaries while relating appropriately with service-users, carers, colleagues, and others.
4. Ensure actions support the equality, diversity, rights, and responsibilities of individuals.
5. Identify, minimize, and report risks of danger, harm, and abuse to senior colleagues.
6. Work within agreed Trust policy and operational procedures.
7. Collaborate with the Royal Stoke Emergency Department and other services to promote seamless service-user-centered care.
8. Plan and undertake delivery of therapeutic activities with service users.
9. Assist in identifying recovery planning that is meaningful to service users and supports agreed health outcomes.
10. Support service-users in meeting their assessed personal care needs.
11. Provide information, support, and guidance to Carers, Friends, and Relatives of service-users, maintaining appropriate confidentiality.
12. Take responsibility for personal and professional development and support the development of colleagues, students, trainees, and volunteers.
13. Participate in activities focused on the delivery of a high-quality customer-focused service.
Person specification
Qualifications
* Advanced Apprenticeship in Mental Health and Social Care or NVQ 3 in substance misuse with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or Functional Skills).
Experience
* Experience of working with people with substance misuse needs as a member of a multi-disciplinary team.
