Job overview
SWB NHS Trust are looking to recruit a motivated and enthusiastic band 6 Senior Practice Nurse within the Summerfield Group Practice.
The successful post holder will be working in accordance with the Code of Conduct, providing assessment, treatment, screening, health education services and advice to patients.
The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and General Practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available. They will also have specialist training for some chronic diseases.
Main duties of the job
Some day to day tasks include:
* Takes responsibility for delegated aspects of outlet management as identified, e.g performance indicators such as QOF and PCCF requirements.
* Day to day relationships will be with other members of the practice team, patients and their carers and other persons visiting or based within the Practice.
* Autonomous practice within the Year of care clinics, ensuring templates are completed in line with QOF & PCCF requirements.

Qualifications
* Registered first level nurse.
* Community nursing specialist qualification or equivalent.
* Mentor/teaching qualification. Clinical supervision training and experience.
* Independent and/or supplementary nurse Prescribing qualification.
Experience
* Relevant experience of primary and community nursing experience.
* Appropriate experience of nurse-led management of chronic conditions.
* Experience of implementing protocols and clinical guidelines.
* Experience of quality initiatives such as clinical benchmarking.
* Experience of Practice Systems clinical systems.
* Experience of data entry.
