Main duties of the job
Manage and promote the repeat dispensing service, repeat prescription reauthorisation, and acute prescription requests. Address medication-related queries from patients and staff. Increase safety mechanisms such as clinical audits.
Implement MHRA drug withdrawals and alerts.
Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication review.
Contribute to reductions in medication-related hospital admissions and readmissions by supporting patients to get the best outcome from their medicines and identifying and addressing medication-related issues.
Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines.
Work within the Group to undertake medication reviews, particularly in high-risk groups such as frail elderly, substance misuse, and patients on high-risk medicines.
Improve prescribing practice through educational support for all prescribers within the practice.
Run and oversee the YMG Anticoagulation service using the surgery's protocol.
Lead on changes in evidence that require changes in prescribing across the patient population, e.g., where a drug is withdrawn or indications change.
Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines.
Attend and contribute to meetings as requested.

York Medical Group (a large, inner-city practice serving a population of approximately 45,000 patients) is looking for an enthusiastic, forward-thinking Pharmacist with a passion for service development, planning for the future, and improving the quality of patient care. The successful candidate will join our diversified team of pharmacists, pharmacist prescribers, pharmacy technicians, and prescription clerks, as well as having the opportunity for multi-disciplinary working with other departments within the practice.
The pharmacy team drives the focus on reducing some of the burdens placed on the GPs by promoting the electronic repeat dispensing service, reducing inappropriate polypharmacy and wasteful prescribing through medication reviews, working with the complex care team to review care home patients, increasing safety mechanisms through audits, implementing drug withdrawals and alerts, and contributing to reductions in medication-related hospital admissions and readmissions by supporting patients to get the best outcome from their medicines.
In addition to the day-to-day duties, the successful candidate will be trained to oversee the anticoagulation service and will also be encouraged to develop in another clinical area of interest. They will also be expected to be proactive, flexible, possess good time management skills, and be able to prioritize workload.

Keep up to date with changes within own area of expertise and ensure appropriate personal development. Review systems for safer prescribing. Contribute to achievement of QoF and locally commissioned QI/cost-effective schemes. Support pharmacy technicians and prescription clerks. Support care homes. Work with the wider team to ensure practices are compliant with CQC standards where medicines are involved.
