REACT (Reactive, Emergency, Assessment, Community Team) brings health, social care and voluntary sector (British Red Cross, Suffolk Family Carer and Citizens Advice Bureau) services together to provide a single coordinated care response that is underpinned by care in the community with early intervention, crisis management, reablement and rehabilitation.
An exciting opportunity has arisen for an Advanced Clinical Practitioner (ACP) to join our award-winning REACT service. Based in the lovely county of Suffolk, this is a key role to support and provide advanced care within the virtual frailty ward.
The virtual frailty ward seeks to manage individuals living with frailty who have an acute medical condition that would usually necessitate an admission to the hospital but with the support provided by the virtual frailty ward team, the individual will be supported to remain in their own home receiving a high level of clinical care, diagnostic and therapeutic interventions to manage the acute episode.
You will be a key player in also building relationships between REACT and primary care, the acute trust, East of England Ambulance Service Trust (EEAST) and the wider community integrated neighbourhood teams.
Main duties of the job
The REACT Services are looking to recruit a fully qualified ACP to work clinically within the community setting with direct responsibility for the Frailty Virtual Ward. You must be a fully qualified ACP with proven experience in a similar role.
The role will provide the opportunity to work within REACT in conjunction with the nurse consultant, Geriatrician or Middle Grade doctor, which will provide a wealth of learning opportunities.
The Advanced Clinical Practitioner role will be integral in supporting the Multidisciplinary Team in the provision and delivery of acute and complex care within the frailty virtual ward.
