As a registered Social Worker in the Hampshire County Council Continuing Healthcare (CHC) team, you'll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. You'll be supported to manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities.
What you'll be doing:
1. Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
2. Undertaking assessments in line with the Care Act 2014.
3. Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
4. Collaborating with individuals and their families to transition them to suitable care provision following assessment.
5. Undertaking mental capacity assessments.
6. Managing challenging and complex cases.
What we're looking for:
1. Social Work qualification and registered with Social Work England (SWE).
2. We would welcome applicants wanting to complete their ASYE.
3. UK based experience within Adult Social Care.
4. Experience and/or knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022 (Revised).
5. A sound understanding of how to apply a strengths-based approach.
6. Understanding of social work issues, legal frameworks and social services' statutory duties.
7. Commitment to demonstrating our values and behaviours.
This is a hybrid role (home and office working) with your office base being the closest Hampshire County Council office to your home address. This role requires travel across Hampshire and working 20% of your week in office. The team meets once a month in Basingstoke.
