Sefton Adult Social Care are looking to recruit a Community Care Practitioner to the Mental Health North Social Work Team based at Hartley Hospital in Southport, to support the delivery of the Adult Community Mental Health Services in the North of the borough. The role of Community Care Practitioner is a non-qualified case worker role within Adult Social Care, and the current vacancy in the Mental Health North Team is for an 18 hour a week part-time post. The Mental Health Team North is co-located on the same site as health colleagues employed by Mersey Care NHS Foundation Trust in the North Sefton Community Mental Health Team (CMHT), working closely with a range of different professionals including Community Psychiatric Nurses, Consultant Psychiatrists, Occupational Therapists and Clinical Psychologists. As a Community Care Practitioner, you would be expected to: Undertake social care needs assessments under the Care Act 2014 with the full involvement of service users, their carers, and representatives. Negotiate appropriate care arrangements with service users and carers including access to the in-house Mental Health Recovery Team, to promote social inclusion following a strengths-based model of community support. Monitor and review the provision of in-house services, placements and packages of care. Support service users to fully utilise in-house and voluntary sector services to maximise their well-being.
