Job summary This is a dynamic time within the addictions and liver cirrhosis team and by joining us you will help to provide patient focused support to an often-marginalized group. This role is to enhance care provision using a bespoke approach to an individual who may be struggling to engage with their community, support services or health care needs. They may also be involved with the criminal justice system and being an advocate for individuals often socially isolated is key. Early intervention and referral to our community partners within addiction will be a further focal part of this role. Tackling Health Inequalities is a key area of this role and improving the patients journey and experience within a hospital setting. This role predominantly sits within the accident and emergency department but also involves in-reach to patients in ward areas and contact with patients who have presented outside of working hours.
