Job Summary
The post holder will be part of the Childrens multi-disciplinary team and will be professionally accountable for demonstrating visible and accessible leadership and Trust values. The post holder will support the Paediatric Multi-disciplinary Team and adult service team to ensure seamless Transition is embedded within the Trust. The post holder will work as a highly specialised nurse co-ordinating with children and young people and the relevant service. This includes facilitating input from supportive services such as Learning Disability or Neurodivergence.
The post holder is expected to exercise a high degree of professional autonomy to manage caseload and make critical decisions to satisfy the expectations and demands of the role. The post holder will support the Childrens Clinical Audit Programme promoting quality improvement initiatives. Act independently within the paediatric team guided by best practice and Trust policies and guidelines. To act as a carer and advocate for families giving expert support and advice based on best practice and close communication with the paediatric and relevant adult service.
To act as a nursing expert in transition both locally and regionally.
PRINCIPLE DUTIES AND RESPONSIBILITIES
Clinical Responsibilities
1. Provide a clinical-led transitional service as a specialised co-ordinator working with a specified group of older children/young adults.
2. Act as an expert resource, giving advice, education/training and support that is appropriately planned to meet the individual's needs empowering them to manage their treatment regimes.
3. Identify potential development opportunities within the transition pathway and work with the Paediatric leadership team to implement these.
4. Project lead and implement change and pathways in partnership and collaboration with other Health Care Professionals.
5. Work within local and national frameworks to implement service changes.
6. Lead on the running of a support group for transitional patients and their family/carers.
Communication
1. Maintain an effective communication network with all multi-professionals, facilitating a multidisciplinary approach to benefit the children, young people and their families.
2. Work with each family individually assessing the level of support they will need using interpersonal skills to build a trusting relationship thus promoting concordance, recognising the dynamics of change for each family.
3. Develop the quality and availability of transition related information for young people and their family units.
4. Act as an expert resource for medical staff both within the trust and outside.
Leadership and Management
1. Contribute monthly/quarterly reports of case mix, workload and education activity which will be presented at Directorate meetings.
2. Represent and report to Trust committees, such as Childrens and Young Persons Board.
3. Participate in the presentation and publication of relevant research and best practice initiatives.
4. Be actively involved with conferences and meetings, developing presenting skills in the regional and national setting.
5. Represent the Trust nationally and network with other Transition Nurse Specialists throughout the UK.
6. Obtain, evaluate and disseminate information to facilitate best practice for children, young people and their families moving between paediatric and adult services.
7. Undertake regular meetings with group and nurse manager to ensure co-ordination of care, continuity and planning.
8. Maintain confidentiality of patient/family and staff information.
9. Undertake all expanded role procedures according to agreed protocols within the scope of Professional Practice.
10. Develop and maintain own clinical skills and knowledge of the transition process that is evidenced based aiming to achieve a 'best practice' principle.
11. Manage time off and delegate care of patients appropriately.
Professional Responsibilities
1. Maintain own professional knowledge development and competence by keeping up to date with advances and research pertaining to Transition.
2. Be aware of the Code of Professional Conduct in the performance of the advanced nursing and be professionally accountable for own actions during the working shift.
3. Manage own caseload managing own time on a weekly and monthly basis.
4. Recognise the level of emotional stress encountered when with patients and their families who are dealing with life threatening and control issues and seek appropriate personal support as required.
Education and Training
1. Act as a role model and clinical expert in speciality when liaising with the local primary healthcare teams and other relevant community health professionals.
2. Participate in education and practice development on a Trust wide basis and as required in a national level.
3. Encourage other staff to recognise the need for nursing research.
4. Be an expert in transition and regularly undertake active teaching and assessing in the education and training of clinical and non-clinical staff.
5. Assess own educational needs and take steps to keep up to date with clinical and nursing developments.
Clinical Governance
1. Support effective governance by disseminating good practice and promoting clinical audit and use of evidence based multi-professional standards.
2. Participate in all aspects of Clinical Governance and Risk Management.
3. Maintain high standards of nursing care within own practice and monitor the standard care for the child or young person requiring admission.
Service Development
1. Ensure the constant provision of effective, high quality nursing care to all patients moving from children to adult services.
2. Take a lead in developing the quality and availability of transition related information for families and community health care professionals.
3. Be actively involved in the development of Transition related protocols and procedures.
4. Participate and be actively involved in multidisciplinary research studies of relevance to the care of young people moving from children to adult services.
