Key Responsibilities: In this role, you will provide physiotherapy assessments, treatments, and rehabilitation to patients in their homes or community settings. You'll manage a diverse caseload, including patients with complex needs, and work collaboratively within a multidisciplinary team. Your expertise will be crucial in promoting patient independence, supporting discharge from hospitals, and contributing to service development and quality improvement initiatives.
