Job Title: Clinical Services Pharmacist - Aylesbury Job Location: Aylesbury, UK Job Location Type: Hybrid Job Contract Type: Full-time Job Seniority Level: Entry level We are looking for a motivated, forward thinking clinical pharmacist to work within the mental health teams, across Oxford Health NHT FT. This post will be based at the Whiteleaf Centre, supporting our pharmacy team to deliver our clinical services. The position offers the opportunity to work with a variety of mental health specialities through a newly planned rotational programme. Specialist clinical areas include inpatient adult, older adult, children and adolescent units and forensic pharmacy. The post holder will have the opportunity to support the Directorate Lead and Clinical Lead pharmacist with innovation of the community-based pharmacy service which is in its infancy. This is a patient facing role and the successful candidate will be working with the other members of the healthcare team on clinical sites. There are times where remote working can be facilitated to ensure efficiency, improve our carbon footprint, to support patient choice and work/home balance. We currently have a small team of clinical pharmacists working within the inpatient teams and a dedicated pharmacist working with our community mental health team in the North of Oxford. We also have a pharmacist dedicated to supporting the community teams across the primary care interface within Buckinghamshire. Our pharmacists work alongside a team of medicines management and medicines optimisation technicians who drive excellence and support our clinical services. The role will offer the opportunity to support the clinical work across these sites as appropriate. We are a looking for a pharmacist who is confident to support our Trust and departmental initiatives to improve patient experience, and promote our clinical and cost effectiveness initiatives. We are a small team working where we can, closely with patients and multi-disciplinary professionals. As a Clinical Pharmacist, You Will Work (physically or remotely) alongside clinical pharmacy colleagues across the Mental Health Directorate. (The ability to travel between sites will be required). Be responsible for the development of strong clinical relationships with the patients, carers and healthcare professionals in your in-patient and community settings. Directly support patients, prescribers, and other healthcare professionals via attendance at multi-disciplinary team (MDT) meetings and one-to-one patient consultations, carry out medication reviews, engage in discharge-planning with the MDT and ensure the correct application of national and local guidance with regard to therapies involving medicines. Support and work alongside our centralised hub pharmacist where necessary (based at the Clinical Pharmacy Support Unit). Support the ongoing development of the community-based pharmacy services. Support the Trust and departmental clinical effectiveness initiatives. Be a member of the on-call pharmacist out of hours team as per rota - where applicable. Be encouraged and supported to professionally develop in line with the departmental and your own personal needs as appropriate. Be supported in continuing professional development through regular clinical and peer group supervision and education sessions within the Pharmacy department and externally where practicable. Have the opportunity to join our in house teaching sessions, alongside our the South West mental health trust collaborators, in order to develop your fundamental knowledge of mental health psychopharmacology and pharmacy practice.
