There is an exciting opportunity for a passionate, experienced cancer nurse to join the Macmillan Lung Cancer Nurse Specialist service, providing high quality patient centered care to those living with lung cancer.
The role involves undertaking a comprehensive Holistic Needs Assessment and care planning at key stages during the illness experience, acting as the patient’s main point of contact coordinating their pathway from the point that lung cancer is suspected.
The post holder will have the opportunity to provide specialist support during outpatient consultations, review inpatients and respond to alerts when lung cancer patients are admitted as an emergency, demonstrating the ability to plan and implement programs of care in response to presenting symptoms.

Main duties of the job
Working as a core member of the lung cancer multidisciplinary team, the post holder will provide high quality patient centred care to those living with lung cancer from the point of suspicion of the disease.
The post holder will act as the main point of contact, managing a caseload of patients, providing specialist information, help and support, liaising with other healthcare professionals within the MDT, primary and secondary care to ensure a seamless service for patients and their families.
The post holder will support the Macmillan Lung Cancer Clinical Nurse Specialists in the delivery of an innovative, high-quality service to those living with lung cancer.
The post holder will be responsible for implementing the key elements of Personalised Care, ensuring patients are offered a Holistic Needs Assessment/care planning at key stages in the pathway in line with national directives.
Detailed job description and main responsibilities
1. To undertake a Holistic Needs Assessment at key stages in the pathway in line with national directives, using a recognised tool, ensuring the patients' physical, emotional, social, financial and spiritual needs are identified.
2. Use supportive conversations to develop and implement plans of care to meet the individual needs of patients/carers, ensuring they are reviewed and adapted in response to the changing clinical situation.
3. To attend the weekly specialist multidisciplinary team meeting as required, contributing to the multidisciplinary discussion and patient assessment/care planning decisions and co-ordinate actions from the MDT.
4. To review patients admitted as an emergency, working with the admitting team to agree and implement an appropriate management plan.
5. To provide advice on the management of symptoms, referring those with complex needs to the Macmillan Specialist Palliative Care Team.
6. To demonstrate enhanced practice in planning, implementing and evaluating nursing care, documenting the outcomes of consultations in the hospital notes and on the Somerset Cancer Database.
7. To provide Level 2 psychological support for patients/carers, referring patients with complex needs to the Macmillan Clinical Psychologist.
8. To lead on patient and carer communication issues and co-ordinate the patient’s pathway, acting as the main point of contact or responsible for nominating the main point of contact for the patient depending on their individual needs.
9. To ensure patients receive specialist verbal and written information at the appropriate point in their illness experience to meet their individual needs.
10. To become acquainted with the local and regional care pathways including knowledge of contact points for referral and advice.
11. To identify and meet the educational needs of those living with lung cancer.
12. To demonstrate knowledge of the clinical trials relevant to lung cancer.
13. Ensure the palliative care needs of patients and their families are identified and met, referring patients with complex needs to the Macmillan Specialist Palliative Care Team.
14. To identify the patient’s Preferred Place of Care at the appropriate point in the illness experience, ensuring patients are able to be discharged to their place of choice where possible.
15. To identify patients suitable for the AMBER Care Bundle.
16. To ensure patients/carers are supported during significant consultations, acting as the patients' advocate.
17. To work in line with the NMC Code of Professional Conduct.
18. To promote collaborative working relationships and effective communication between all members of the cancer multidisciplinary team and across different boundaries of care.
19. To provide nursing advice and support to other health professionals related to lung cancer.
20. To identify own personal development needs to work at specialist level and take appropriate action to ensure that these are met, actively engaging with the learning and development opportunities offered by Macmillan Cancer Support.
21. To maintain a personal professional profile clearly demonstrating evidence for revalidation.
22. To participate in the formal review process and plan future personal development and competencies to be achieved.
23. Make sure that people affected by cancer are aware that they are interacting with a Macmillan Professional and know the full range of resources and services available through Macmillan Cancer Support.
24. To record nursing interventions on the Somerset Cancer Database.
25. To record data to demonstrate the effectiveness of the Macmillan Nursing Services.
26. To deputise for the Macmillan Clinical Nurse Specialists at local, regional and national meetings.
27. Participate in Macmillan Service Reviews in order to drive service improvements.
28. To participate in the self-declaration against national cancer standards and external peer review visits.
29. Implement the key elements of personalised care and support working with the Macmillan Personalised Care Transformation Lead.
30. To support the Macmillan Clinical Nurse Specialists in the development of policies/clinical guidelines to enhance the nursing service.
31. To participate in informal/formal education initiatives related to lung cancer.
32. To participate in clinical audit and research initiatives.
33. To promote best practice ensuring care is evidence based.
34. To contribute to the management of the service.
Person specification
* Educated to degree level in cancer or palliative care or equivalent level of education, training or experience.
* A recognised teaching qualification or equivalent level of education or training.
* Recognised communication skills training.
* Evidence of continued professional development.
* Recent experience in caring for patients living with lung cancer.
* Experience of multi-professional working and management of own caseload.
* Understanding of national frameworks related to cancer.
* Understanding of Personalised Care & Support.
* Understanding of Macmillan Cancer Support and its role across the UK.
