Job summary
We are looking for a practice nurse to join our friendly, supportive and committed Practice Team. The successful individual will be responsible for the delivery of practice nursing services, working as partof the practice multidisciplinary team, delivering care within their scope ofpractice to the entitled patient population. The practice nurse will beresponsible for a number of clinical areas such as health promotion, chronicdisease management and health prevention. A relocationpackage of £8,000 if moving from outof area and 5 weeks annual leave pro rata
Main duties of the job
1. Support patients with long term conditions.
2. Perform treatment room procedures
3. Undertake Cervical Cytology
4. Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines, including Dopplers and removal of sutures
5. Carry out as required new Patient Medicals/Urinalysis
6. Offer lifestyle advice as appropriate.
7. Undertake ECGs
8. Undertake Venepuncture
9. Support patients with Chronic disease management, undertaking annual reviews and follow-ups.
10. Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions
11. Carry out Routine immunisations/Childhood immunisations
12. Provide Chaperoning and assisting patients where appropriate who are being examined by another clinician
13. Assisting GPs with minor surgery and other clinical procedures
14. Requesting pathology tests, for example urine culture, swabs
15. Following agreed clinical protocols with referral to senior nurses or GPs as appropriate

31. Maintaining and cleaning equipment used by the nurses and GPs

Learning and Development

All staff will be required to partake and complete mandatory training as directed by the training coordinator, as well as participating in the practice training programme.
