We are looking to recruit a highly motivated, forward thinking, committed and enthusiastic Nursing Associate with excellent communication skills to join our Tissue Viability Team. You will be pivotal in helping us to develop this new and exciting role within our team and act as a role model to current and future trainee nursing associates.
You should have excellent clinical, written and verbal communication skills and have completed the Trainee Nursing Associates course, gaining NMC registration.

The nursing associate role has a breadth of knowledge across the lifespan of patients and across the fields of nursing; providing holistic and person-centred care and support for people of all ages and in a variety of settings. The nursing associate works independently under the leadership of registered nurses who work within the sphere of nursing and care.
Nursing associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a nursing associate and will work to a nationally recognised code of conduct.

Main Duties and Responsibilities
1. Provide and monitor care in the community setting:
1. Provide compassionate, safe and effective care and support to people in a range of care settings.
2. Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers.
3. Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment.
4. Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.
5. Recognise and report any situations, behaviours or errors that could result in poor care outcomes.
6. Undertake lower limb re-assessments and wound management using the appropriate documentation following the full training and assessment of own competence levels by a regulated professional.
7. Record and analyse the following clinical tasks using existing clinical knowledge and experience; Measure and record blood pressure, temperature, height & weight – body mass index, Purpose-T assessments, nutritional assessment using Must tool.
2. Planning:
1. Working alongside senior health professionals in planning the clinics and visits, prioritising according to agreed protocols and seeking advice when required.
2. Participating in co-coordinating patient visits and planning reviews accordingly.

4. Promoting Health and Preventing Ill-Health:
1. Support people to improve their mental and physical health and wellbeing.
2. Active involvement in the prevention of and protection against disease and ill health.
3. Engage in public health, community development, and in the reduction of health inequalities.
Communication:
1. To liaise as appropriate with members of the Adult community teams.
2. Develop good working relationships and liaise as appropriate with parents/ carers, and other health, social or voluntary agencies.
3. To communicate effectively with clients in complex situations. Communication skills of engagement, negotiation, motivation and explanation will be required to enable the management of potential barriers to communication.
4. To be actively involved in the communication of complex information regarding clients’ needs and any other useful information to the Adult Nursing Team, and/or other appropriate medical services (such as GP) and members of the wider Adult Services team.
Budgetary Responsibility:
1. Take responsibility to minimise incurred expenditure e.g. Mileage, telephone calls use of equipment etc.
2. Ordering and receipt of stock.
People Management:
1. Actively participate in annual performance reviews providing evidence of attaining the relevant knowledge and skills required for the job.
2. To attend mandatory and relevant training courses and to maintain up to date clinical knowledge and skills in order to perform in the role.
3. To participate in staff and service development through attendance at service meetings and in service training.
Clinical and Practice Governance:
1. Observe and maintain strict confidentiality with regards to any information in line with the requirements of the Data Protection Act.
2. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.
3. The post holder must adhere to the Trust risk assessment and risk management processes.
4. To contribute in programmes and education support for staff and students.
5. To actively participate in programmes of supervision to maintain own development and reflective practice.
6. To actively engage in developing own practice through annual clinical observations.
7. The post holder must adhere to infection control policies and procedures.
8. The post holder must participate in clinical and safeguarding audits as required.
9. The post holder is required to participate in relevant emergency preparedness process for their team.
