To provide a comprehensive, specialised nursing service to patients in relation to colorectal conditions, stoma care and fistula management.
Acting as a key member of the multidisciplinary team, advisor and expert resource to other health professionals within the hospital and community Trusts.
Main duties of the job
1. Provide expert advice and support to patients and carers from diagnosis and throughout the patient pathway.
2. Provide clear, accurate and appropriate written and verbal information according to the patients needs.
3. Act as a point of contact, as a coordinator of care, liaising with MDT and others.
4. Attend MDT and other relevant meetings, contributing specialist nursing knowledge and skills and acting as a patient advocate.
5. Assess, plan and implement care for individual patients.
6. Responsible for assessing, negotiating and making optimal site for stoma formation in patients undergoing elective colorectal surgery.
7. Educate patients and relatives about basic principles of stoma care.
8. Provide ongoing support.

* Proven experience at Band 5 with an interest within colorectal / stoma care.
* Demonstrable awareness of current speciality issues.
* Awareness of Local and National drivers relating to NHS and Oncology pathways.
* Experience of working within a clinical area that provides services for colorectal lower GI patients.
* Experience of working with learners and delivering clinical based education / development.
Desirable
* Experience of working within a Band 6 Role previously either substantive, secondment or development.
* Experience in audit projects.
Qualifications
Essential
* RN Adult (NMC).
* Willingness to commence and complete Degree or Master Degree (if degree already completed).
* Certificate in Teaching/Assessing - or equivalent experience.
* Evidence of recent post registration training and development relevant to current post.
