Job summary Chet Valley Medical Practice located in Loddon, are pleased to be recruiting a pharmacy technician to join our experienced in-house team on a part-time basis for 30 hours per week. The successful individual will have daily access to an experienced clinical pharmacist plus regular support and supervision sessions. Main duties of the job Undertake medicines reconciliation in accordance with practice protocols and to update patients medical records/prescribing accordingly. Carry out medicines optimization tasks including effective medicine administration and technical medication reviews. Utilize consultation skills to work in partnership with patients to ensure they use their medicines effectively. Maintain full and complete records of all patient contacts using appropriate clinical templates and coding. Support medication reviews and medicines reconciliation for new care home patients. Support the achievement of the practices prescribing Quality and Outcomes Framework targets. Working with the dispensary manager/ clinical pharmacist, contribute to the review and development of repeat prescribing protocols /standard operating procedures to improve the efficiency and effectiveness of repeat prescribing systems.
