Working alternative weekends. The Weekend On Call Care Coordinator plays a vital role in ensuring that HHC Care Team are Supported in the Community and that all Domically Care Calls are covered. HHC On Call service is a telephone service available between 6 a.m. and 10 p.m.
The ideal candidate will possess strong organisational skills and a keen attention to detail, ensuring that all aspects of care are managed efficiently.
Whilst you will be Home Based you will need to be available throughout your shift to support your team and deliver Care in the Community when required.
Option of additional Community Care hours available weekdays.
Duties
* Serve as the primary point of telephone contact for Carers and Clients.
* Addressing inquiries and concerns regarding care services.
* Updated schedules to cover any sickness or absences. Liaising with Care Team.
* Community Care - Including Personal Care, Medication, Food and Drinks.
Essential Criteria
* Previous experience in Domiciliary Care
* Minimum 3 Years Care experience
* Knowledge of Scheduling
* Strong organisational skills
* Understanding of Safeguarding and how to respond .
* Excellent communication skills, both verbal and written, with a compassionate approach to client interactions.
* Proficiency in using office software.
* Ability to work collaboratively within a team while also being self-motivated.
* A commitment to maintaining client confidentiality
Desirable Criteria
* Level 3 Qualification in Health and Social Care.
