An exciting opportunity has arisen for anexisting or newly qualified band 5 to obtain a role in the Crisis and HomeTreatment Team. We will support the successful candidate through induction andcompletion of preceptorship.
Crisis and Home treatment Team provide crisisassessment and home treatment to patients in acute mental health crisis in theleast restrictive environment. CAHTT also work closely with inpatient servicesto facilitate early planned discharge.
The Crisis & Home Treatment Team alsomanage the Hatton Suite for those detained on a S136 of the MHA; and alongsidethis we offer an overnight Liaison service to Wye Valley Trust.
The role includes assessment and hometreatment in locations to include the patients home within the Communityand/or treatment rooms based at the Stonebow Unit.
Crisis & Home Treatment Team operate a 24hour service 7 days per week. We are looking for a dynamic and enthusiasticindividual who is able to work hours per week, over 3/4 days/nights (12hour shifts)

Main duties of the job
It's alittle unpredictable, as you can be called at any time to a crisis situation,we discuss referrals each morning and three times weekly we review our caseloadwithin our MDT.
Wecomplete comprehensive risk assessments for every patient, these can becompleted independently although sometimes we discuss more complex patients asa Multi-disciplinary team.
Weliaise with families and carers, GPs, community mental health teams, socialcare teams, community nurses, physiotherapists and voluntary organisations, andanyone who is necessary to maximise recovery.

Undertake full psycho-socialassessments with those in need of such. Ensuring thorough, precise and timelydocumentation and dissemination of information in line with Trust policy andprocedure.

Essential
7. Ability to communicate clearly and effectively in English in both verbal and written formats.

12. Demonstrable post- qualification experience in acute mental health
13. Undertaking multi-disciplinary assessments
14. Undertaking mental state and self-harm assessments
15. Assessing and managing risk
16. Working within the Care Co-ordination framework
Desirable
17. Experience of assessing people who deliberately self-harm and those diagnosed with personality disorder
18. Experience of working with people who engage in substance misuse
19. Experience of working with/assessing people with cognitive impairment and/or functional mental illness
20. Working with user and carer groups
Knowledge

21. Good working knowledge of Mental Health Act legislation and Mental Capacity Act 2005.
22. Clinical Governance, Risk Assessment and Adult/Child Protection Legislation.
