The role of Mental Health Practitioner is to work in partnership with children and young people in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. This role is for 22.5 hours per week.
The post holder will demonstrate an understanding of clinical interventions relevant to the care group, including education, care plans, and discharge planning, working closely with carers, families, and partner agencies to facilitate this.
The post holder will work at all times to promote the safety and well-being of children and young people and their families/carers. They will assess, plan, and implement care, and provide specialist advice and interventions. Additionally, they will provide clinical supervision to staff and students and act as a mentor for students.
Main Duties of the Job
The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of children and young people by undertaking the following duties:
1. To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those children and young people presenting with higher levels of risk. To develop care plans alongside the child or young person.
2. To offer specialist assessment and advice where required.
3. To assess and manage ongoing risks as identified during the assessment, ensuring that this is done in collaboration with the child, young person, and carers. To regularly review risk factors and make changes to the management of them as necessary.
4. To contribute to the maintenance and development of the Trust Nursing and Mental Health Strategy.
5. To ensure that clinical practice is evidence-based and consistent with relevant NICE guidelines and Trust policies and procedures.
6. To provide supervision of junior staff and trainees where appropriate.
7. To act within the professional body's code of conduct at all times.

Person Specification
Qualifications
* Registered professional with current registration, appropriate to the job role (Social work, Teachers, Occupational Therapists, and Creative Therapists).
* Diploma/ Degree in relevant profession.
* Completion of Mentorship Course.
* Evidence of continuous professional development.
Experience
* Around 2 years post-qualifying experience (preferably in the relevant care group).
* Experience of working alongside people with complex health problems in the designated care group.
* Experience of supervising staff.
* Experience of managing a caseload.
* Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems.
* Experience of working consultatively with professionals.
* Experience of conducting clinical assessments including risk assessments.
* Experience of partnership working within and across various statutory and non-statutory teams and agencies.
* Experience of working within the CPA process.
* Understanding of clinical governance.
* Previous experience of working in the community.
* Experience of contributing to the implementation of services.
