Job summary We are offering an exciting and unique new role as a Band 4 Senior Health Care Support Worker within Stafford Integrated Mental Health Team (IMHT) on a 12-month Fixed Term Contract. We are also offering this role as a secondment opportunity. We are looking for someone who is motivated and curious to effectively and positively interact with individuals with co-occurring needs, who are or have been homeless. This role will further support these individuals through sustained and consistent engagement to hopefully working towards interventions which may improve their mental wellbeing. You will be working with some key partner organisations and in collaboration with staff at Holly House, House of Bread and Crisis House to provide oversight of a individual's recovery and the contribution that you can make. This role provide a fantastic opportunity to work autonomously, to make it your own and to be instrumental in us working more effectively and collaboratively with other agencies both in statutory and voluntary sectors to provide a dedicated and consistent presence to facilitate effective engagement and progressfor those with co-occurring needs. Main duties of the job You will be working closely alongside our partner organisation Holly house. holly House is a 6 bedded supported accommodation staffed 24/7. Holly House provides crucial support to six service users with complex mental health and co-occurring needs who are at risk of disengaging from standard MPFT services. As part of this role you will be expected to: Work directly with those residing at Holly House contributing to a recovery approach to enhance their wellbeing. Be curious and motivated to make a difference to this cohort of individuals with co-occurring needs. Have the necessary knowledge and skills of those with co-occurring needs to identify and provide wherever possible specific interventions around a framework of engagement bring together all services to enhance the offer.
