A exciting opportunity has arisen to join our primary care teamwithin Sutton Primary Care Networks (PCN) as a Team Managerproviding support to GP practices for patients presenting with mental health problems.
The service looks at psychological, social, biological and environmental factors that may be impacting mental health and looks to support service users through early intervention, relapse prevention and management of mental health, using a range of approaches to achieve this.
The post holder will work as part of the primary care teamwithin Sutton Primary Care Networks (PCN), providing support to GP practices for patients presenting with mental health problems. This support will include provision of advice, education, support and interventions to patients and carers. It also includes offering expert advice and education to primary care staff across the four Primary Care Network(s) in Sutton PCN. The PCN Mental Health Practitioners Manager will provide effective day-to-day management of the service, including people (workforce) management, co-ordinating own and team workloads, to ensure referrals are processed and initial contacts made within agreed timeframes. Use Business Intelligence (Trust + PCN dashboards) to monitor performance and implement mitigations where necessary and within allocated resources. Lead by example (values) and contribute to the NHS England/Improvement & Trusts vision to transform the way we deliver Community Mental Health Services (improving Access, Recovery, Reduce pressure on Acute services, and improve the Patient Experience). The PCN Mental Health Practitioners Manager will also represent the team internally / within the Trust as well as externally with our stakeholders.

· Be responsible for the smooth and effective operational management and co-ordination of the Sutton PCN Mental Health Practitioners Team
· Continuously review & improve systems for referral management and treatment resources management, ensuring a fast and appropriate response to referrers
· Hold regular meetings with the team, to ensure that the separate elements of team accountabilities are fully addressed.
· Hold full managerial responsibility for all staff members, ensuring they work within an agreed operational policy
· Ensure Data Quality by delivering comprehensive inductions to new starters (and regular update/refresher training), monitoring performance information and ensuring records are completed in a timely & appropriate manner.
· To facilitate and enable appropriate communication within Primary care networks within a specified borough, between other teams and departments across South West London including Local Authorities, service users, carers and other relevant agencies.
· Regularly review and manage incidents reported by the team, and where appropriate support/lead on the investigation of Complaints, under the guidance of the Trusts’ or PCNs Quality & Governance Dept.
· Be available for assessments or other clinical work where unplanned circumstances deem this to be the priority i.e. “available for action” in the event of an emergency.
· Lead and participate in the management of workforce (Vacancies & Recruitment), using Dashboard appropriately to process Payroll Variations, Requests to Recruit through to shortlisting & interviews (TRAC)
· Be actively involved in multi-disciplinary meetings, including safeguarding and to ensure that safeguarding of Children and Adults is appropriately considered and managed at all times.
· Liaise with the managers of the Sutton PLRS/IRH/HTT/Talking Therapies as required.
· Ensure comprehensive liaison and feedback to GP’s and Primary Care Network referrers.
