As a dedicated Through Care Team Social Worker, you will play a pivotal role in supporting children and young people transitioning from care to independent living.

As a seasoned Social Worker, you will work collaboratively with multi-agency teams to provide holistic support, including education, health, and housing services.
You will also provide advice and advocacy for young people, ensuring their voices are heard in decision-making processes.
Responsibilities:
* Manage a caseload of young people transitioning from care to independent living.
* Conduct assessments of young people's needs, identifying areas where additional support is required.
* Develop and implement pathway plans to ensure the best possible outcomes for young people leaving care.
* Work in collaboration with multi-agency teams to provide comprehensive support.
* Provide advice and advocacy for young people, ensuring their voices are heard in decision-making processes.
Requirements:
* Qualified Social Worker registered with Social Work England.
* Experience in working with young people, particularly those transitioning from care to independence.
* Strong knowledge of relevant legislation, including the Children Leaving Care Act.
* Excellent communication, assessment, and report-writing skills.
* Ability to work effectively with young people and professionals from various agencies.
