We have an exciting opportunity for a new role across the whole of Peterborough supporting the uptake, training and provision of annual health checks for people with learning disabilities.
You will be at the forefront of improving the uptake and quality of annual health checks for the county's learning disability population.
You will have a wealth of specialist experience relevant to working within community based multi-disciplinary teams supporting people with learning disabilities. You will have experience of advocating for people with learning disabilities especially around access to primary health care.
You will contribute effectively to the ongoing development of the service in line with National Policies and evidence-based practice. As an autonomous practitioner, you will manage your time in relation to the health needs of adults with a learning disability. You will work closely with the lead nurses to promote and support your role.
Main duties of the job
* To work to improve the uptake and quality of the Annual Health Checks for the Learning-Disabled population across Peterborough under the guidance of the lead Nurse.
* To participate in multi-professional/multi-agency working within the LDP/Peterborough Learning Disabilities team and with Key partners - particularly the GP surgeries within the C& PCCG.
* To provide ongoing support and liaison to the GP practices within the clinical area which will include providing training on LD and validating their QOF learning disability register to support the uptake of annual health checks.
* To link in with MDT professionals in the LDP/Peterborough Learning Disabilities team to support reasonable adjustments to facilitate Annual Health checks.
* To devise a questionnaire to measure the quality of the AHC and lead on evaluation of results.
* To identify those with or without consent to share information and offer information for the person to decide.
* To work with the relevant brokerage teams to ensure provider services have a good awareness of the value of annual health checks.

Job responsibilities
The post holder will demonstrate specialist knowledge and skills. Understand Health and Social care policy and legislation for the service user group in relation to your professional practice.
* To provide advice and support to service users, carers and partner agencies regarding annual health checks for people with learning disability.
* To liaise and communicate effectively and sensitively with other professionals in the MDT and external agencies (e.g. GP surgeries, the CCG, private providers) providing highly specialist advice as necessary.
* To improve the uptake and quality of the annual health checks locally.
* To work effectively as part of a multi-disciplinary team and across partner agencies.
* Lead in the delivery of training to GP surgeries across the area, linking in with Peterborough Nurses from that locality area.
* To have a good understanding of the physical health needs of the service user group.
* To support reasonable adjustments that may be required to enable service users to access their AHC including recognising the impact of a persons communication needs and tailor interaction to meet this need.
* To have skills in managing difficult conversations as the role requires working with surgeries where there is poor uptake of AHC or poor quality AHCs are being reported.
