In partnership with the Family Support Service Manager, you will play a crucial role in developing a comprehensive framework and strategy designed to deliver an effective Social Work service tailored to meet the needs of patients with frailty and life-limiting conditions. Your focus will be on providing skilled interventions to these patients, ensuring that they, along with their families and carers, receive the necessary support during challenging times. The role will involve advocating for frail patients, helping them achieve the best possible health outcomes and quality of life. You will manage a defined caseload that includes Supportive Calls, collaborating closely with the Family Support Service Manager to ensure individualised care. As a key member of the team, you will work alongside the Designated Safeguarding Lead as the Adult Safeguarding Champion, contributing to the development and maintenance of Safeguarding Adults at Risk and Safeguarding Children policies in collaboration with the Children & Young Peoples Safeguarding Champion. You will also be responsible for creating and implementing policies and procedures that facilitate access to hospice services and relevant external resources for patients and their families. Additionally, you will assist in the development and delivery of mandatory safeguarding training and educational initiatives in partnership with the Education team, ensuring that all staff are equipped to provide safe and compassionate care. Your support will extend to both the in-patient unit and community services, where you will provide formal and informal assistance to colleagues within the multidisciplinary team. You will also play a role in mentoring and managing service volunteers. Throughout your work, you will embody the hospices philosophy and values, striving to enhance the lives of frail patients and their families facing the realities of palliative care and life-limiting conditions.
