Birmingham and Solihull Mental Health NHS Foundation Trust
An exciting opportunity has arisen for a full-time Band 6 Primary Care Mental Health Practitioner to join our Solihull Neighbourhood Mental Health Team.
Within BSMHFT, our community mental health services are well-established. As part of the BSOL community transformation programme, we are looking to increase our primary care teams with primary care practitioner roles. You will be responsible for providing mental health triage, assessment, and outcomes-based intervention to meet service users' needs within the Primary Care Network. You will identify needs and the best course of advice, support, or intervention, promoting psychosocial wellbeing by identifying and supporting pathways for service users and carers.

2. Promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.
3. Support and advise colleagues within the PCN and across local neighbourhood multi-agency teams.
4. Ensure robust communication across mental health providers, VCSE organisations, and within primary care.
