Job Title: Social Worker - Adult Community Team Location: Lincolnshire Rate: Up to £35 per hour An exciting job role has recently become available in North Lincolnshire. They are looking for a new member for their Adult Community Team. What will your responsibilities be? You will effectively manage a caseload with varying complexity, providing a statutory range of assessments, reviews, and reassessments using social work methods, theories, and models to identify actions that achieve change and development, improving life opportunities. Working within all relevant legislative requirements, policies, and procedures, you will assess and meet the needs of adults and their circle of support, providing appropriate support through the offer of a personal budget where necessary. Maintaining accurate, comprehensive, and comprehensible records in accordance with applicable legislation, protocols, and guidelines is essential. You will arrange and chair planning meetings, customer reviews, and other ad hoc meetings as required. Working closely with partners and other agencies, you will assess, plan, arrange, and coordinate a range of innovative services to meet the needs of adults and their circle of support, ensuring best value and qualitative arrangements are in place. Providing appropriate and effective supervision to allocated non-qualified team members is also part of your role. If required and as directed, you will undertake duties within Home First, responding to initial referrals and implementing short-term pieces of work to prevent the need for longer-term social work interventions. You will attend multi-disciplinary and multi-agency forums, promoting effective cross-agency working practices and recognizing diversity. Additionally, you will participate in safeguarding enquiries as required, liaising with multi-agency partners and taking appropriate action within legislation, reporting to stakeholders as necessary.
