An exciting opportunity has arisen for you to join the Health Support team in Tameside.
You will be based at Trust HQ but will work at sites across Tameside where Depot clinics and Clozapine Clinics are held.
The post is a progression post from Band 5 to Band 6, dependent on achieving a set pre-planned competencies. There will be an expectation of working towards achieving these competencies in a time frame of around 1 year.
Key Responsibilities:
1. To administer intramuscular injections, monitor the effect of medications, contraindications and side effects, and provide accurate information to patients and carers in a format they are able to understand.
2. To undertake venepuncture training and maintain knowledge and skills to be able to use this in both clinical and residential settings.
3. To be able to use the POCHI machine and follow processes outlined in Clozapine Clinic procedures.
4. To work with patients within the CMHT’s/EIT as indicated, participating in the planning, delivery, and evaluation of their individual programme of care, to address presenting health and wellbeing needs, in conjunction with any other relevant professionals.
5. To undertake physical health checks, and respond to & report concerns appropriately.
6. To provide health promotion for service users in the Stockport Mental Health Service, understanding the relationship between mental health and physical health.
7. To refer onto expert services in the community where a need has been identified.
8. To enhance a patient's overall wellbeing, and promote parity of esteem.
9. To undertake any other duties, appropriate to the grade, that may be required.
