JOB PURPOSE.To provide pharmaceutical support to the Practice to support safe and effective prescribing under the guidance and direction of the Lead GP. Prescribe medication for patients within agreed protocols. Provide pharmaceutical advice to patients to enable them to get the best from their medicines. KEY RESPONSIBILITIES To reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge. To provide pharmaceutical medication review and health education advice to patients at the designated practices by face to face, telephone or by notes-based review as agreed with the practice. Make clear and comprehensive records of any interventions etc in patients EMIS records so that they are available to other practice staff. To provide pharmaceutical input to long term conditions clinics as appropriate To hold medication review clinics and facilitate implementation of changes with appropriate follow-up monitoring Print FP10MDA instalment prescriptions to ensure they meet legal requirements following the agreed protocol (scripts to be signed by GPs and not by pharmacist). To develop and run pharmacist-led clinics to monitor the effects of medication by checking clinical signs and symptoms where medicines are the main intervention. E.g. minor ailments, anticoagulation, asthma, diabetes or blood pressure. Support processes for repeat prescriptions for care homes that are looked after by SGMC. To participate in continuing professional development and work towards an Independent Prescribing qualification. To contribute to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes To act as a source of medicines information for all of the practice team and patients: e.g. around doses, side effects, adverse events, possible alternatives (around out of stocks). To improve prescribing practice though educational support for all prescribers within the Practice. Work with prescribers, local pharmacists, patients and their carers to arrange dosette boxes / other compliance aids. To lead on areas where changes in evidence require changes in prescribing across patient populations e.g. where a drug is withdrawn or indications change. To support improvements in clinical care through practice based audits and implementing change To liaise with community pharmacy regarding transfer from care settings and the use of monitored dosage systems. To provide a telephone helpline for patients with questions, queries and concerns about their medication. (Repeat prescription requests will usually be signed by a GP but the pharmacist may prescribe an item from the patients repeat if it is within their clinical competence to do so and circumstances deem this the best course of action see Protocol for Pharmacist Prescribing.) To manage and support the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates where required. To monitor adherence to the repeat prescribing policy ensuring repeat medication reviews are conducted at the appropriate intervals To maximise the potential of the appropriate use of the repeat dispensing service. To promote electronic prescribing services within the practice To implement drug withdrawals and alerts, e.g MHRA alerts, aimed at improving medicines safety. Support the development of policies, clinical protocols and guidelines in the practice Drafting reports summarising status on issues, appraising outcomes, and providing progress reports for the Lead Medicines Optimisation Pharmacist and Practice GP Prescribing Lead Collate as required, qualitative and quantitative information and lead appropriate analysis to develop and contribute to projects Analyse, interpret and present data to highlight issues, risks and support decision making. Liaises with the CCG Medicines Management Team on prescribing related matters to ensure consistency of patient care and benefit
