Main duties of the job
Key Responsibilities:
1. Chronic Disease Management (Diabetes, CHD, Asthma/COPD)
2. Women's Health, including cytology
3. Vaccinations and childhood immunisations
4. Leadership and mentorship of the nursing team
5. Contribution to service development within the practice
6. Working closely with GPs and other healthcare professionals

Job responsibilities
Assist in and perform routine tasks related topatient care as directed by clinical leads and GPs. Such tasks include:
7. Conduct chronic diseasemanagement clinics, including diabetes, asthma, COPD, and hypertension.
8. Perform cervical cytology (smeartests) and provide sexual health advice.
9. Administer vaccinations andimmunisations, including childhood and travel vaccines.
10. Provide wound care, includingdressings and suture removal.
11. Conduct health checks andscreenings.
12. Urinalysis
13. Spirometry
14. Phlebotomy
15. Ear Irrigation
16. Chaperoning and assistingpatients where appropriate who are being examined by another clinician
17. Requesting pathology tests longterm condition management
18. Following agreed clinical protocolsas appropriate
19. Processing of samples
Other taskswill include:
20. Clean and sterilise equipment asappropriate
21. Maintaining stock and rotatingitems as required
22. Participation in healthcampaigns, flu campaign
23. Maintain general tidiness andcleanliness of nurses rooms
24. Maintaining and ordering healthpromotion literature
25. At all times there will be aneed to maintain accurate records
26. Ordering of vaccination tomaintain stock levels
27. Participate in administrativesystems in the Practice
28. A duty to advise clinical leadsand the Practice Manager of potential problems or errors within range ofassigned tasks
29. Attend and participate in anyPractice meetings when required
30. Any other delegated dutiesappropriate to the post
31. Undertake annual training anddemonstrate competency in anaphylaxis and resuscitation techniques

Person Specification
Qualifications
Essential
41. Registered Nurse RN with current NMC registration.
42. Competence in chronic disease management, wound care, and immunisations.
43. Strong organisational and time-management skills.
44. Ability to work independently and as part of a multidisciplinary team.
45. Excellent communication and interpersonal skills.
Desirable
46. Nurse Prescriber desirable but not essential.
47. Experience in a General Practice or primary care setting is desirable
