The Clinical Coordinator for Out of Area Patients will be responsible for monitoring those DHC patients who are placed in adult acute, PICU, and non-acute out of area (OOA) specialist beds under the Named Patient budget, due to the lack of appropriate local provision.
The position is a six months fixed-term / secondment covering three days a week in the absence of the current post holder. You will work collaboratively along side the current post holder.
This role will be responsible for ensuring the delivery of high quality person centred services with a focus on integrated care which always considers people’s safety, privacy and dignity. To monitor effectiveness of placement and ensure it is continually meeting the patients’ needs.

Identify and implement changes to clinical practice in the service area ensuring the delivery of up to date evidence based practice.
This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
