The post holds many development opportunities and offers a chance to manage an autonomous caseload and plan care around patient-centered goals.
Main duties of the job
The successful candidate will work the hours over 5 days, Monday to Friday, and manage a caseload of cardiac rehabilitation patients within the community setting. The service is clinic-based with occasional home visits. You will also work on the ward providing stage 0-1 of the commissioning pack pathway and in community exercise classes.
We are a NACR accredited service.

* Two years post-registration experience, 1 of which must be in recent CVD care

Qualifications
* Registered Nurse (Level 1)
* Current NMC registration
* Relevant experience/qualification or training in CVD
* Relevant teaching and assessing qualification
* Car owner
* First Degree (or working towards)
* Independent Nurse Prescribing qualification (or willingness to work towards)
* Experience of working within primary care
* Experience of Cardio exercise programmes
* Cultural awareness, experience of working with ethnic minority groups
* Knowledge/understanding of Primary Care
* Experience of previous service development
