Main duties of the job Key responsibilities This will be varied and will adapt over time as both the ICB/S develops. However, the postholder is expected to fulfil the following key functions: - To provide specialist nursing assessment, procurement, case management and review for adults and young people in transition to adult services presenting with complex long-term care needs, who are eligible or potentially eligible for NHS Continuing Health Care or Funded Nursing Care or section 117 aftercare services. To provide assurance around quality of commissioned care packages, reporting safeguarding concerns in accordance with agreed interagency reporting procedures. To carry a case load of the most complex client group, including adults who have experienced an acquired brain injury and /or require medical ventilation or tracheotomy care. To ensure safe discharge and transfer of individuals from highly supportive and acute care environments to community placements avoiding out of area placements.
