An exciting opportunity has arisen to lead the experienced team of Cardiac Rehabilitation nurses and exercise therapists at the Great Western Hospitals NHS Foundation Trust. The Cardiac Rehabilitation programme delivered by the team has current British Association for Cardiovascular Prevention and Rehabilitation (BACPR) GREEN certification status.
You should have excellent communication skills and the ability to work independently as well as part of a multidisciplinary team. Your vision and leadership will drive you to further develop this service to meet the individual needs of cardiac patients along with their families/carers.

The post holder will be responsible for providing effective clinical leadership supporting a team of nurses and therapists ensuring the Nursing & Midwifery Council (NMC) code of conduct is at the forefront of the team's nursing care. The position encompasses a clinical role in addition to the management of operational issues and the provision of expert health advice in relation to cardiac rehabilitation.
The post holder will lead and manage the development of the Cardiac Rehabilitation Service, coordinating an inter-disciplinary team to provide support, information, and guidance to the patient, their partner and/or family, and other health professionals. The post holder will also implement and evaluate rehabilitation programmes which are integrated as part of the patients' ongoing care in both the hospital and community settings.
The post holder will work closely with the multi-disciplinary team in the provision of a high standard of individualised patient care, thus improving the patient experience. The post will incorporate the provision of care to patients, their families, and carers in accordance with the philosophy and standards of care within the clinical area.

Key Responsibilities:
1. Act as a clinical expert in their area of practice.
2. Work clinically as part of the Cardiac Rehabilitation team, sharing experience, knowledge, and expertise.
3. Using specialist skills and knowledge, identify and assess patients' suitability for Cardiac Rehabilitation and refer to other professionals as appropriate.
4. Manage own caseload of inpatients and outpatients.
5. Work clinically as an autonomous practitioner in virtual and face-to-face Cardiac Rehabilitation clinics.
6. Order appropriate pathology testing, ECGs, and echocardiography.
7. Demonstrate evidence-based physical assessment and clinical evaluation skills.
8. Work as an Independent Nurse Prescriber within the Scope of Practice for the role.
9. Offer specialist information, advice, and support to patients/carers from the time of diagnosis, throughout treatment and follow-up.
10. Analyse and respond to complex clinical situations utilizing specialist knowledge.
11. Signpost and refer patients to other health professionals or supportive services.
12. Complete accurate and legible documentation to record cardiac assessments/interventions.
13. Provide good written communication to the patients' GP and patient regarding individual assessments and behaviour changes/goals agreed.
