The North Locality Team is seeking to appoint an enthusiastic and motivated individual to the position of Agency Social Worker, in the Adult Social Care & Homelessness Service, Denbighshire County Council.
The successful applicant will work within a team of Social Workers, Occupational Therapists and social care staff. This temporary post will be located within the Rhyl Community Resource Team (CRT), where health, social care and third sector teams are co-located, to deliver integrated services to the local community.
A commitment to positive and effective partnership working is a core requirement of the role.
The post holder will undertake appropriate and proportionate assessments to enable individuals and their families to achieve realistic and desirable outcomes.
A Professional Qualification in Social Work and registration with the Care Council for Wales is essential. The post holder will be working as part of a multidisciplinary team and must have the drive, energy and self-motivation to prioritise and manage own workload.
The post holder will be able to work flexibly, and the team offer blended working, from home and from the CRT office base.
You must have:
1. A professional qualification in social work
2. Social Work England registration.
3. Enhanced DBS/Update service.
4. References required for the last 5 years of employment.
