As a Care Coordinator, you will play a vital role in supporting both staff and patients, by coordinating and supporting their care and support across the NHS services. This role involves working closely with general practice teams (including other PCN professionals) within the PCN to help support patient caseloads ensuring their needs are met. You will assist them to manage their health by accessing the necessary services. Additionally, you will handle administrative tasks such as appointment management and for the recording of meeting minutes ensuring all discussions and decisions are accurately documented. The goal is to deliver personalised care, improve patients' quality of life, and contribute to a seamless healthcare experience. This non-clinical role requires a caring, dedicated, and proactive individual with strong organisational and communication skills.
Main duties of the job
Patient Care Coordination:
1. Develop and review personalised care plans with patients, families, and carers. Identify patients needing additional support, particularly those with complex needs.
2. Assist with appointments, information-sharing, and patient queries to empower informed care choices. Collaborate closely with Primary Care colleagues for cohesive care.
Multidisciplinary Team (MDT) Support:
1. Participate in MDT meetings to coordinate care for at-risk patients. Act as a communication hub, ensuring that all involved professionals are aligned. Refer patients to appropriate services and support networks.
Administrative Support:
1. Handle key administrative tasks, from recording meeting minutes to managing patient records. Support with meeting coordination and data management.
Patient Advocacy and Support:
1. Guide patients in managing their health, accessing educational resources, and participating in shared decision-making.

* NVQ Level 3 or equivalent level of knowledge in office procedures.
* Experience of working as a lead administrator, knowledge of office procedures.
