Job overview
An opportunity has arisen to join our stoma therapy team. The post holder will work with the stoma care nursing team in delivering and managing inpatient and community care. They will provide specialist services and advice to patients and their families who potentially or actually require stoma forming surgery. Core roles will involve teaching and supporting patients to acquire self-care skills enabling transition from hospital to community and providing ongoing specialist support services in the longer term as required for patients with any type of stoma.
Main duties of the job
* Accept referrals to see patients from consultant surgeons, ward staff and other members of the multidisciplinary team.
* Site for stoma formation in the optimum position to prevent physiological or psychological distress to the patient.
* Provide patients with advice on suitable appliances and accessories as required.
* Be responsible for own caseload of patients with new or established stomas within the hospital and provide continued advice and support on discharge.
* Plan and prioritise working day to best meet the needs of the service, whilst remaining flexible enough to respond to sudden changes in those needs.
* Contact and meet with patients preoperatively to provide information and education to them and their relatives/carers regarding their proposed surgery and stoma formation.
* Provide postoperative information, counselling, and support to ensure that the patients and their relatives/carers have a level of knowledge commensurate with their needs.
* Offer a supportive service applying specialist knowledge to enable patients to adapt to their surgery and stoma formation.
* Assist and advise ward teams with all aspects of care for patients with a stoma.
* Promote safe and effective care of patients by having awareness of, and utilising Association of Stoma Care Nurses (ASCN) UK Clinical guidelines to underpin evidence-based practice.

* Registered Nurse
Experience/Qualifications
* Degree in Nursing, or willingness to undertake top up. PS/PA or mentorship qualification or equivalent. Evidence of continued professional development in related field. Significant experience in relevant specialty. Significant nursing experience in hospital setting, and interest in stoma care.
* Qualification in stoma care. Colorectal module. Experience of undertaking audit, research and evidence-based practice.
Skills
* Knowledge of evidence and research-based practice relating to stoma care. Excellent interpersonal skills both verbal and written. Ability to communicate effectively and discuss sensitive and sometimes highly complex and potentially distressing information with patients and families/carers. Teaching and facilitation experience. Ability to participate in and implement the development of nursing practice. Ability to demonstrate commitment to high quality patient care and patient experience. Able to problem solve and seek solutions.
