We would expect you to - Act as a role model to what outstanding care should look like. Support and mentor clinical staff to deliver person centred care. Provide family and carers with assurance that their loved one is being supported and looked after. Work within a supportive management team to improve patient journey through secondary care services. Undertake clinical audits and screenings Provide Line Managed Supervision, including in depth caseload reviews. Supporting safe and effective discharges. Providing medication reviews. Undertaking assessments as and when required. Supporting with 72 hour follow up.
