We're looking to bring on board an organised, enthusiastic, self-motivatedand driven individual to join our community care advice team and work closelywith hospital discharge teams and community services to deliver value-adding NHS& Social Care services. The core objective of the Care Advisor role atUBH will be to support individuals, their families and their representatives tofind the right care, ensuring the best possible outcomes for the care seeker,in partnership with the care provider market and support of our health andsocial care partners. The role will require you to work alongside Health &Social Care Professionals, meeting key performance indicators that improvepatient outcomes. Main duties of the job
Supporting individuals and families at what can be a stressful time to find the right care
Work closely with hospital discharge teams to identify individuals requiring support
Work in partnership with care providers to source appropriate care
Follow up with all relevant stakeholders to ensure a smooth, stress free process
Attend hospitals and community events to promote the service and drive referrals
Ability to work independently, with great attention to detail and the ability to think proactively is essential
Post holder will be confident in using computers daily, including competent use of word processing, spreadsheets, databases and e-mail, as well as daily diary management software and the internet.
Maintain accurate and up to date records on our CRM, liaising with health and social care staff to ensure that information is provided and updated where necessary.
To monitor e-mail systems and exercise discretion to ensure that urgent and essential issues are brought to the team's attention. Proactively respond to and or re-direct other e-mails to relevant managers for action/information where appropriate.
To build effective working relationships and work as part of a small team.
Ability to deal with interruptions and unplanned events as necessary, reprioritising work accordingly when required.
To promptly refer any concerns about adult safeguarding or child protection to an appropriate person.

Pay close attention to detail (, proofreading), and ensure that all outgoing and internal material reflects a high standard according to company policies and the NHS identity principles. All material should be accessible, with consideration given to barriers to understanding, such as a first language other than English, learning difficulties, or other impairments, including visual and hearing.
To deal with enquiries from staff in health and social care organisations relating to referrals
To deal with enquiries from patient representatives, carers and care providers.
To maintain the highest standards of sensitivity and confidentiality when dealing with information about individuals.
Pre-populate documentation to aid appointments for Health & Social Care Professionals
To carry out other appropriate delegated duties as required.
